Your Path to APCM Readiness Starts with HCUnity
2025 – Getting Started
APCM begins. New codes (G0556–G0558) each month, rather than billing separately for CCM or TCM. You cannot bill APCM and CCM/TCM in the same month. RPM can still be billed if all rules are met.
Annual Wellness Visit (AWV) gets stronger. A Social Determinants of Health (SDOH)check (G0136) can be added and paid separately. Advance Care Planning (ACP) stays billable.
Quality rules start. CMS sets a core list of measures (Universal Foundation).
2026 – More Clinics add APCM
2027 – Focus on Data Digital Reporting, More Programs Universal Foundation Measures
2028 – Building Out AWV Framework and APCM flow electronic quality reports. ACOs have to report more Universal Foundation measures.
2029 – Proving Results APCM, plus RPM, should now be routine for the right patients. Practices should be showing clear monthly records of care coordination, medications, goals, and follow-ups
2030 – The Big Goal, CMS wants all Medicare patients to be in some form of “accountable care”. No single big penalty date, but pressure grows through incentives and program design.
