Our Story

HCUnity was founded in 2023 after years of leading digital health initiatives across hospitals, private practices, care management organizations, and technology companies. No matter the setting, the same problems kept surfacing—disconnected platforms, overpriced tools, inefficient workflows, and technologies that added work instead of removing it.

We’ve walked in these shoes before. From running full-service care management programs to building healthcare platforms used nationwide, we’ve seen what works—and what doesn’t. Drawing on decades of experience, we set out to create something different: a platform rooted in real-world healthcare challenges, designed to adapt and grow alongside CMS’s evolving value-based care programs.
