Like many families, ours faced the realities of chronic illness firsthand.

Gary’s father lived with diabetes for years, carrying the daily burden of a progressive disease that ultimately led to blindness, limb loss, and kidney failure.

Meta’s father battled Idiopathic Pulmonary Fibrosis, a condition that slowly took away his ability to breathe and live independently.

And both of our mothers faced cancer.

At the time, care management programs like Remote Patient Monitoring (RPM), Chronic Care Management (CCM), Transitional Care Management (TCM), and Advanced Primary Care Management (APCM) either did not exist—or were not accessible in ways that could truly support families like ours.

But looking back, we know something clearly:

They would have benefited from this.

That realization became the foundation of HCUnity.

With decades of healthcare technology experience, more than 12 years working directly in care management operations, and years spent building care management solutions, we have seen both sides of the challenge.

The personal side—families navigating chronic illness, caregiving, uncertainty, and loss.

And the operational side—healthcare organizations working to coordinate complex care through disconnected systems, fragmented workflows, and limited visibility.

That experience shaped our understanding of what was missing.

HCUnity was built to bridge that gap.
